Our program is committed to providing individuals who have mental illness with additional services to become functional in their homes & community. Our program’s main goal is to: • Link individuals with mental health services (e.g. outpatient counseling and medication management) • Ensure clients follow treatment recommendations of providers • Coordinate services for physical health & wellness checkups • Conduct ADL’s and IADL’s independently
